Parental / Guardian Consent for: • Acknowledgement of My Child to Participate  in Juneteenth Mentorship Expo
 

I understand that this activity involves travel to and from Eckerd College. I understand that my child’s participation in the activity is a privilege, and not a right. I acknowledge that I have spoken with my child about my child’s need to comply with the specific rules and requirements established for this activity. I understand that all rules apply to my child and the other students during the course of the Juneteenth Mentorship Expo.

The undersigned further agree to indemnify and hold Tables Of Purpose, Inc. and their respective members, directors, employees, and agents (collectively, the "Indemnities,") harmless from and against any and all claims, demands, actions, lawsuits, and liabilities, including attorney fees and expenses and costs sustained by the Indemnities as a result of negligent, willful or intentional acts of the undersigned and/or participant (if participant is 18 or under).

The undersigned do hereby release, forever discharge and agree to hold harmless Tables Of Purpose, Inc, from and against any and all liability, claims, demands, lawsuits and expenses of any kind arising from personal injury, sickness (including COVID), death or property damage of any kind whatsoever which may be incurred or suffered by the undersigned and/or participant (if participant is 18 or under).

TRANSPORTATION PERMISSIONS AND WAIVER

I also understand that private drivers, which may include my child (pending my written permission below), or the parent of another student participating in the activity, may be used to transport students to and from the activity. The owner of the vehicle must carry bodily injury insurance. Tables Of Purpose, Inc.  does not cover damages arising from, or related to, the operation of any private vehicle, failure to follow the directed driving route, or any personal negligence related to this activity. Any damages/harm resulting from a parent/guardian/or other designated driver (including student-drivers), arising from the operation of a motor vehicle in relation to the above listed activity, is hereby waived. I give permission for my child to ride in a vehicle to and from the activity with another student. I give permission for my child to transport students to and from the activity.

 I, the parent or legal guardian of the participant, do hereby grant permission for my child to participate fully in the Juneteenth Mentorship Expo and all of its activities. In the event that I cannot be reached, I hereby give permission to the agents of Tables Of Purpose, Inc to call medical support.  I assume all responsibility and medical costs. 

 

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
